Another gatefold jacket... Episodes 3 and 4 are on this disc. Mistao Katsuragi (葛城ミサト) is the cover-girl this time.

Here are both inserts. The cover art sheet was limited to the first 1,500 copies of each LD. The other insert has information printed on both sides, but I really didn't want to photograph all the inserts twice... sorry!
